Last spring, Eastland-Fairfield successfully provided an opportunity to nearly 400 individuals to receive the Pfizer-BioNTech COVID-19 vaccine. In an effort to provide additional opportunities for our students, families, and staff to combat the effects of COVID-19, we have decided to host a second clinic to offer the FDA-approved Pfizer vaccine.
NOTE: This is a vaccine for those not yet vaccinated. This is NOT a booster.
Eastland-Fairfield is partnering with the Fairfield Department of Health to provide a clinic at Fairfield Career Center in Carroll.
Clinic 1: Thursday, September 16
Clinic 2: Thursday, October 7
Both clinics will be held between 8:00 a.m. and 12:00 p.m. in the FCC Multi-Purpose Room.
All high school students enrolled at Eastland-Fairfield Career & Technical Schools, including satellite, all students enrolled in Adult Workforce Development programs, and any EFCTS staff are eligible to receive the vaccine. The clinic is being provided at absolutely no cost.
Please complete the fillable consent form and bring that form with you on the first day of the clinic.
High school students must have a parent or guardian authorize the form with their signature. Please bring this form with you to the clinic, return it to your school nurse in advance, OR scan and email a copy of the consent form to your child's school nurse.

WHO IS ELIGIBLE
- EFCTS students from Eastland and Fairfield campuses in grades 11 and 12
- EFCTS students from any of its satellite locations
- EFCTS students enrolled in any Adult Workforce Development program
- All EFCTS staff members
- Family members of the above groups (12 years of age or older)
This clinic is free to those that register.
This clinic is completely voluntary.
Parent attendance is not required to receive the vaccine.
There is no requirement to provide proof of insurance to receive the vaccine.
Transportation will be provided for Eastland Career Center students if needed.
Satellite students must provide their own transportation to and from the clinic.
By completing the registration you commit to receiving both vaccine doses on those prescribed dates.
Vaccination cards will be made for each student after the first dose is received; however, Eastland-Fairfield will hold those cards until after the second dose is administered. Vaccination cards will be released to the student following the final dose. Staff members will receive their vaccination card following the first dose and need to bring it for the administration of the second dose.
Students that are driving on their own from the Eastland Career Center or their satellite school must sign out before leaving campus and are expected to return.
Students using provided transportation from the Eastland Career Center will leave at a designated time (to be provided later) and will be returned to campus following their vaccination.
Vaccinations will be completed on a first-come, first-serve basis. Please note that this may produce wait times to receive your vaccine.
As of August 24, the Pfizer vaccine has received full FDA approval and is the only COVID-19 vaccine to have received this designation. If you have any questions regarding the vaccine, please contact your child’s healthcare provider.
